Osseous Surgery / Pocket Reduction

1

Improving Gum Health with Flap Surgery, Osseous Surgery, and Guided Tissue Regeneration
When periodontal disease advances, the gums begin to detach from the teeth, forming deeper pockets that trap bacteria and become difficult to clean. Over time, this leads to bone and tissue loss, putting your teeth at risk. Pocket reduction procedures are designed to eliminate these deep spaces, reduce inflammation, and support long-term oral health.

What Is Pocket Reduction?
Pocket reduction refers to a category of surgical procedures used to access and clean deep periodontal pockets and reshape or regenerate the supporting structures of the teeth. These procedures may include flap surgeryosseous surgery, and guided tissue regeneration, depending on the condition of your gums and bone.

Flap Surgery (Periodontal Surgery)
Flap surgery involves gently lifting the gum tissue to gain access to the root surfaces and underlying bone. Once the area is exposed, Dr. Sakhai thoroughly removes plaque, tartar, and inflamed tissue from the pocket.

After cleaning, the gum is repositioned snugly around the tooth to eliminate the deep pocket. This reduces harmful bacteria and makes it easier for you to maintain your gum health at home.

Osseous Surgery (Bone Recontouring)
In some cases, advanced gum disease leads to irregular bone contours that trap bacteria. Osseous surgery is often performed along with flap surgery and involves reshaping the bone around the teeth to eliminate these defects. By smoothing and contouring the bone, Dr. Sakhai creates an environment where the gum can heal properly and fit more tightly around the tooth.

Guided Tissue Regeneration (GTR)
If significant bone or tissue loss has occurred, guided tissue regeneration may be recommended. This technique places a biocompatible membrane between the gum and bone to prevent the soft tissue from growing into areas where bone should regenerate. Sometimes, a bone graft or biologic material such as PRF (platelet-rich fibrin) is placed to further stimulate healing and regeneration. This approach encourages the body to rebuild the bone and periodontal ligament that support your teeth.

What Are the Benefits?

  • Reduces pocket depth and bacterial buildup
  • Helps stop the progression of periodontal disease
  • Saves natural teeth that might otherwise be lost
  • Restores a healthier gumline
  • Facilitates long-term maintenance and hygiene

Is This Right for Me?
Pocket reduction procedures are often recommended when non-surgical therapies such as scaling and root planing have not fully resolved inflammation or when pockets remain too deep to clean effectively. During your consultation, Dr. Sakhai will evaluate your gum condition using clinical measurements and 3D imaging to determine the most effective surgical approach for you.
