What Is Silver Diamine Fluoride?
Silver Diamine Fluoride (SDF) is a topical liquid used to slow or stop the progression of dental decay. It combines the antibacterial properties of silver with the enamel-strengthening effects of fluoride, making it an effective treatment for cavities—especially when traditional restorations may not be possible or ideal. SDF has been approved by the FDA for treating dentinal sensitivity but is commonly used off-label in the U.S. to arrest active dental caries, particularly in children, older adults, and patients with special healthcare needs.

When Is SDF Recommended?
SDF may be recommended for: 

  • Young children with early childhood caries 
  • Patients with difficulty tolerating traditional treatment (e.g., due to anxiety, medical conditions, or special needs) 
  • Decay in areas that are hard to restore 
  • Geriatric patients with root caries or limited access to dental care 
  • Arresting cavities temporarily while awaiting definitive treatment 

What to Expect During Treatment 

  1. The tooth is gently dried and isolated.  
  2. A small amount of SDF is applied to the decayed area with a microbrush. 
  3. The material is left for 1–2 minutes and then allowed to dry. 
  4. No drilling, numbing, or filling is necessary during this visit. 

Multiple applications over time may be needed for full effectiveness. 

Risks and Potential Side Effects
While SDF is a safe and effective treatment, it does have some limitations and considerations: 

1. Permanent Staining of Decay 

  • The most notable side effect is black staining of the decayed portion of the tooth. 
  • This is cosmetic only and indicates that the decay has been successfully stopped.  
  • Healthy enamel and surrounding gum tissue are unaffected. 
  • Front teeth may be more noticeable when treated with SDF, and cosmetic alternatives may be discussed.

2. Temporary Gum Irritation 

  • Mild, temporary irritation of the gums or soft tissues can occur if SDF touches them during application. 
  • This typically resolves within 24–48 hours. 

3. Metallic Taste 

  • Some patients report a mild metallic taste, which fades quickly. 

4. Not a Permanent Solution 

  • SDF halts decay but does not restore the lost tooth structure. 
  • In some cases, a filling or crown may still be needed later for strength or aesthetics. 

5. Allergy (Rare) 

  • Allergic reactions are extremely rare but possible. Be sure to tell your provider if you have a known sensitivity to silver.

Is SDF Right for You?
Silver Diamine Fluoride is a valuable tool in managing dental decay, but it may not be appropriate for all situations. Your dentist will evaluate the location, severity, and cosmetic concerns before recommending it as part of your treatment plan.
